I am aware of and expressly assume all risks (known and unknown) associated with attending the Extra Life Kick-Off Party being held at the SickKids Foundation - Ted Garrard Boardroom (525 University Avenue, 14th Floor, Toronto, ON, M5G 2L3) on March 26, 2026. I agree that if I am buying an Event ticket on behalf of other guests, I will ensure and I represent and warrant, that each Event guest on whose behalf I purchase an Event ticket, has read and agreed to this Waiver, Release and Indemnity, which shall apply to each of them as if the reference to “I” was a reference to them. I, on behalf of myself and my personal representatives, assigns, heirs and next-of-kin, hereby waive, release and forever discharge The Hospital for Sick Children Foundation, The Hospital For Sick Children, any beneficiaries, sponsors, other Event participants, advertisers, consultants, participants, third-party vendors, government or public entities, and each of their respective affiliates, successors, officers, directors, members, employees, volunteers, agents, and representatives (collectively referred to as the "Releasees"), of and from any and all proceedings, claims, demands, damages, costs, expenses, actions and causes of action (whether known or unknown) whatsoever, both in law and in equity, which I now have or may hereafter have for or by reason of or in respect of attending the Event and notwithstanding that same may have been contributed to, or occasioned by, the negligence of any of the Releasees.
I further agree to hold harmless and fully indemnify the Releasees from any and all actions, causes of actions, claims, damages and demands whatsoever which may be brought against or made upon the Releasees and against all loss, liability, judgements, costs or expenses which the Releasees may sustain, incur or be put to resulting from or arising out of any act or omission on my part or of my guests, agents, successors or assigns which was done, or purported to have been done, in connection with my attendance of the Event.
I agree to conduct myself in a safe manner and to adhere to all federal, provincial, and municipal laws, including without limitation any and all public health guidelines and measures regarding the prevention and mitigation of spreading infectious diseases such as influenza, respiratory syncytial virus (“RSV”), or other viral respiratory infections, including COVID-19. I agree to drink responsibly and that the Releasees are not responsible or liable for any damages, including but not limited to, property damage, personal injury and death to myself or third parties that result from the consumption of alcohol by myself or others at the Event.
I acknowledge and agree that attending the Event may increase the risk of being exposed to or infected by any influenza, RSV or other viral respiratory infections, including COVID-19, and any influenza, RSV or other respiratory infections, including COVID-19, may result in personal injury, illness (such as fever, cough, sore throat, shortness of breath or difficulty breathing), hospitalization, permanent disability, and death. I, on behalf of myself and any of my guests, voluntarily assume all risks and dangers associated with influenza, RSV or other viral respiratory infections, including COVID-19 that may result from attending the Event.
The provisions of this Waiver, Release and Indemnity shall enure to the benefit of the respective heirs, executors, administrators, personal representatives, successors and assigns of each of the Releasees and shall be binding upon my heirs, executors, administrators and personal representatives. This Waiver, Release and Indemnity shall be interpreted and the rights of the parties determined, under the laws of the Province of Ontario and Ontario courts shall have exclusive jurisdiction for any dispute.

I understand and acknowledge that I am giving up substantial rights by accepting the terms of this Wavier, Release and Indemnity. I agree and acknowledge that I am signing this Waiver, Release and Indemnity freely and voluntarily, and intend by my agreement to be a complete and unconditional release of all liability against the Releasees to the greatest extent allowed by law. If any portion of this Wavier, Release and Indemnity is held to be invalid, the balance notwithstanding, shall continue in full force and effect
